For current patients creating an account, a few important tips: * Do not create an account for the parents, only accounts for each child individually. * Create your first child’s account. You will be asked to complete a demographics page. Have insurance card information ready. It will take about 10 minutes. * Next, after you create your first child’s account, go to “My Patient Page” and click on “manage my family account”. You will then choose to add another dependent and create accounts for each of your other children. We will ask for a separate login/password for each child, but the accounts are linked so you will only need to use your first child’s login/password for all of your children’s accounts. * When you wish to use the site to make a request, notice that your first child’s login/password that you used to login is the ''myself'' listed under “Active Account”. Use the drop down menu to change active account from myself to any other of your children for whom you wish to make a request. *Please complete all required fields otherwise we will not be able to print it out prior to your visit.
