We collect information you voluntarily submit through forms on our Site, including:

      New patient enrollment forms (demographic information, insurance details, emergency contacts)

Each of these vendors has its own privacy policy, and where they handle your health information, they are bound by HIPAA Business Associate Agreements with our practice.

5. Cookies and Tracking Technologies

5.1 What Are Cookies

5.3 YouTube Video Embeds 

Some pages on our Site include embedded YouTube videos. We use YouTube’s privacy-enhanced mode (youtube-nocookie.com), which means YouTube’s servers are not contacted and no cookies are placed on your device until you actively choose to play a video. 5.4 Google Maps

Some pages on our Site include an embedded Google Map to display our office location. Google Maps may set cookies and collect your IP address when the map loads. Our Site is intended for use by parents and legal guardians of our pediatric patients. It is not directed at children under the age of 13, and we do not knowingly collect personal information directly from children under 13 through this website.

In accordance with the Children’s Online Privacy Protection Act (COPPA), if we become aware that we have inadvertently collected information from a child under 13 without verified parental consent, we will delete that information promptly.

If you are a parent or guardian and believe your child has submitted personal information through our Site without your knowledge, please contact us at admin@rtcpeds.com and we will remove the information as quickly as possible.

All patient health records for minor patients are maintained in our clinical systems under the direction of a parent or guardian and are governed by our HIPAA Notice of Privacy Practices, not by this website policy.

8. Data Retention

      Form submissions (contact, info and career forms): retained for 3 months or until the inquiry is resolved, then securely deleted.

This Privacy Policy governs the manner in which Connexin Software, Inc. dba Office Practicum collects, uses, maintains and discloses information collected from you as a visitor (each, a “User”) to a Office Practicum, NextStep Solutions, Remedy Connect OP Site or other web- or cloud-based sites offered by Office Practicum (collectively, “OP Site(s)”). This privacy policy also applies to any products and services available through Office Practicum portals, affiliate websites, or forums, or services offered by Connexin Software, Inc. dba Office Practicum (collectively, “OP Service(s)”).

We may collect information from Users in a variety of ways, including, but not limited to, when Users visit, access, register, fill out a form, or otherwise use OP Sites or OP Services. Users may be asked for, as appropriate, name, email address, mailing address, phone number, company name, etc. (“personally identifiable information”). Users under 13. Protecting the privacy of the very young is especially important. For that reason, we never collect or maintain information at OP Sites from those we actually know are under 13, and no part of our website is structured to attract anyone under 13.
